Prior to ANY audit being performed, you must submit the following Request Form to your Regional COR Department:

When preparing for your Audit, please become familiar with the 2019 New Audit Standards

If you have not already done so, be sure to register for an Audit Lunch & Learn which is a requirement for all current Valid Auditors (check the course calendar for available dates in each region).


Please contact your Regional COR Administrator for access to the Audit (Regular COR or SECOR)

It is the responsibility of the Auditor/Assessor to submit any required documents to MHSA (ie: Request forms)

MHSA will be closed as of Noon on Tuesday, December 24th, 2019 and will re-open after the holiday season on Thursday, January 2, 2020.

Please adhere to Audit Submission deadlines:

Certification Audits – by Nov. 30/19

Maintenance Audits – by Dec. 24/19

Peer Qualification Audits – by 6 months from course date

All Certification Audits must be assigned by MHSA – companies are not to select your own auditor as we have strict guidelines to adhere to PIR standards.
