Prior to ANY audit being performed, you must submit the following Request Form (click here) to your Regional COR Department. Please be sure to include all sites.

When preparing for your Audit, please become familiar with the 2019 New Audit Standards
If you have not already done so, be sure to register for an Audit Lunch & Learn which is a requirement for all current Valid Auditors (check the course calendar for available dates in each region).
IMPORTANT THINGS TO NOTE:
Please contact your Regional COR Administrator for access to the Audit (Regular COR or SECOR)
It is the responsibility of the Auditor/Assessor to submit any required documents to MHSA (ie: Request forms)
MHSA will be closed as of Noon on Tuesday, December 24, 2019 and will re-open after the holiday season on Thursday, January 2, 2020.
